Seymour Risk Assessment
Each company, organisation and/or individual that hires the Seymour Centre MUST provide a risk assessment for the event to be presented in the venue. This document should detail the specific risks and controls associated with the event.
If the organisation does not have a risk assessment, then the company representative must read and electronically sign the Seymour risk assessment below.

This form should then be e-mailed to James Stuart, Safety Officer firstname.lastname@example.org Questions: 02 9351 7946
HAZARD IDENTIFICATION RESOLUTION PROCEDURE
In accordance with OH&S WHS Legislation an employer has a responsibility to provide and maintain so far as is practicable for employees a working environment that is safe and without risks to health. Similarly, all employees have a responsibility to take reasonable care for his or her own health and safety and for the health & safety of anyone else who may be affected by his or her acts or omissions at the workplace.
As such the Seymour Centre encourages the identification and reporting of workplace hazards or incidents. A hazard can be identified at any time, by anyone. Once identified, the risk must be assessed (see Appendix A in document) and controlled in line with the hierarchy of controls (see Appendix B in document).
